Most of human history had barely any knowledge of science compared to today. If someone was born very ugly / disfigured, it's quite plausible that the other villagers / tribe members would come up with a superstitious reason to explain it. Possession by a demon would be a likely one during Christian times. Before that they could easily be cast out by fearful villagers due to them being a "monster", probably leading to a hermit life in the woods. Why do think trolls and ogres are always depicted as living alone, in the woods or under a bridge? Many incels were probably killed out of fear too, but more would appear in each new generation due to bad luck of the genetic lottery.
It's not a coincidence that evil is very often associated with ugliness, and beauty with good. Most monsters that heroes had to slay in most myths were horribly ugly. Ugliness was often an indicator of disease (good reason to stay away) or simply bad genetics and not someone you'd want to reproduce with.
Basically there are deep cultural, historical, mythological, scientific and genetic reasons why people especially women are repulsed by ugliness. "Just get a haircut + shower bro, just get a good personality" doesn't even scratch the surface of the problem.

Incels even show up on opera. Alberich in Das Rheingold, from the race of ugly chthonic Nibelungs, tries to seduce the Rhine Maidens at the beginning of that opera, and they find it oh so funny to lead him on and reject him.
He gets even by renouncing love so that he can steal the Rhine Maidens' gold, forge the Ring of Power from it and try to rule the world.

Come to think of it, the Giants in Das Rheingold are incels too. They built Vahalla for the gods based on the understanding with Wotan that he would give them the hot goddess Freia as payment. Freia finds the Giants repulsive, however, and Wotan has to talk them into accepting the Nibelungs' gold as a substitute, though they also demand the Ring of Power to close the deal.

I did some research on this last semester actually and I found it really interesting. In the early modern period they called it "Teratology", the study of monsters. It was a fairly rudimentary science that beleived that pregnant women were the cause of their babies deformations. So for example, practising witchcraft while pregnant leads to babies with missing fingers, or eating lots of blackberries while pregnant causes purple birthmarks. Shit like that. But the terminology is really striking to me, how a deformed child could be born and the initial reaction was to call it a monster, find somebody to blame and then start studying it.

People have always been scared of difference and when you add to that a lack of education and exposure to the outside world, even today we can see huge discrimination based on what people THINK of another who is different.

Yes and no. Yes because The original Lucifer was an angel, and was beautiful by default. Him going against God’s command was a big deal because he was/is an angel.
No, because later renditions literally demonized him into this giant, red horned monster with a large sexual appetite. Whether you tie this to the fall from heaven or not is up to you. I’m not a huge follower of religious mythology, so it’s difficult to say when we split the two apart. It doesn’t help hat we sometimes combine the two, resulting in a (asexual)beauty with asexual appetite.
